For Returning Patients
If you have any change of information regarding your demographics or insurance plan, please contact our office so we may update your information as soon as possible. You can also update this through our portal.
Scheduling an Appointment
Appointments are available Monday through Thursday 8 AM to 11:30 AM and 1 PM to 4 PM, Friday, from 8 AM to 11 AM. The office is closed for lunch from 12 PM to 1 PM. Patients should arrive 30 minutes prior to their scheduled time if they have not completed the initial registration online. If you do not register for our online portal, you may print and complete these forms from here.
Please bring the following information to every appointment:
- Insurance Card (please note: If the patient is not the policy holder, please also bring the Date of Birth and Social Security # of policy holder)
- Copay, Deductibles and Coinsurance will be collected at time of service
- A Photo ID
- Medications (either a list of the names and dosages, or the bottles)
- Name and phone number of your preferred pharmacy
- Name, Practice Name and phone number of your referring provider and/or primary care provider. This will ensure good communication between our office and your healthcare providers.
Canceling an Appointment
While we understand that there are times that we cannot predict timing, we require a 24-hour notification of cancellation. This allows us to see another patient that has been waiting for an appointment. If you cannot come, please call us as soon as you are aware, so we can reschedule your appointment to accommodate your needs.
There is a $100 fee for any non-cancelled appointments. We ask for a minimum of 24 hour notice to reschedule.
Laboratory facilities are conveniently located in our office and offered by Solstas/Quest. Those services are provided separately and independently from our practice, therefore any services or billing issues will need to be directed to Solstas/Quest. Their customer service number is 866-697-8378.
Please have your pharmacy send us an electronic refill request or fax. You may also use our portal for refill requests.
Please allow up to 7 business days to receive and review your test results. If you have signed up for a portal account, your results will be sent to you through that account. You will receive an email notification to log in to view your results.
Medical records request
To release your records, a signed authorization is required. There is a release form on this site.
After Hours and Weekends
For an endocrine emergency that cannot wait until the next business day, please call our office at (919) 571-3661. Choose option #8 for the provider on call. Please do not use this line for refills and/or appointment requests.
